How We Help: Planning and Navigation Packages

Advance Care Planning is the term used in healthcare for the planning required  to ensure your wishes and values are honored by medical personnel (such as doctors, nurses, and ambulance teams).  Advance Care Planning includes completing documents such as a living will and healthcare power of attorney.  It may also include completing a POST form (POST stands for Physician Orders for Scope of Treatment) and other documents.  It often includes conversations with your loved ones to make sure they understand your wishes and can represent them if you are sick.  We assist you with completion of these forms in a manner that reflects your wishes and values. 

We offer two packages to complete your advance care planning - our Healthcare Planning Comprehensive Package and our Healthcare Planning Update Package.  Our Comprehensive Package is for those who have not completed advance care planning before or those who believe they need significant changes to their existing documents and communication around their wishes.  The Update Package is for those who want to revise existing advance care planning documents to ensure they are updated and reflective of your current wishes and circumstances.  Our Advance Care Planning Packages ensure that your plans are informed by your unique medical situations - we help you consider how your health realities, along with your wishes and values, will impact various options available to you. 

The Healthcare Planning Comprehensive Package includes:

• Review of your medical conditions and history to ensure your healthcare planning is personalized for you. 
• Overview of advance care planning documents structure, purpose, and content.
• Step-by-step explanation of components of healthcare planning documents with real world examples of how these documents can be utilized. 
• Discussion of your values and wishes to ensure your documents reflect you.
• Draft version of documents for your review and editing to ensure they accurately represent your wishes. 
• Discussions with you and your surrogate decision maker(s) and/or power of attorney to ensure they understand your wishes and can represent them accurately.
• Completion of advance care planning documents 
• This is completed over 3 virtual 45-minute sessions

The Healthcare Planning Update Package includes:

• Review of your medical conditions and history to ensure your healthcare planning is personalized for you. 
• Review of existing healthcare planning documents (including living will, power of attorney, POST form)
• Discussion of wishes and values to ensure your documents are aligned  
• Discussion of updates you want to documents and considerations we suggest, as well as contemporary considerations to ensure documents reflect your wishes/values
• Completion of updated documents and review to ensure accuracy
• This is completed over 2 virtual 45-minute sessions 

COST: $695

We specialize in helping patients and loved ones navigate medical care when facing serious illness and even end-of-life.  Medical treatments and systems are increasingly complex and it can be a frustrating maze of appointments, tests, treatments, and more.  We assist you to better navigate care, understanding a road map that takes into consideration your unique realities and wishes, and giving your the confidence that you are getting care aligned with your wishes and values.  We specialize in assisting those with complex and serious illness, those experienced reduced quality of life, and those considering options such as palliative care and hospice.  We provide deep insights into your situation so you get the care you need in the way you want it.  

Our healthcare navigation and advocacy requires a minimum of 2 virtual 45-minute sessions with a Board Certified Patient Advocate; additional sessions are available and will depend on your needs and desires. 

Examples of what we will provide you:

• Clarity into your illness trajectory and possible pathways you could take
• Preparation for doctors visits so you have key questions and discussion points outlined in advance
• Advice on care frameworks (including curative, palliative, hospice, and others) that are available 
• Connecting you with medical practitioners in your area that can meet your needs
• Communicating your wishes and preferences to your existing healthcare team
• Assistance for contingency planning for upcoming decision points and care horizons
• Helping you articulate your wishes and priorities to define what you want out of your healthcare
• Clarifying confusing or contradictory information you are getting from healthcare providers
• Providing insight into "what's really going. on" with your health situation (let's cut through complicated terminology and get a real sense of things)
• Navigation and advocacy assistance based on your questions and desires (services are customized for each client and situation)
